Together, with all schools in the KHSD, all 9th and 11th grade students will participate in the California Healthy Kids Survey (CHKS) during the last week of February through the first week of March. The CHKS provides important data to the state, district and school. It asks questions about school climate and safety, health practices, drug and alcohol use, violence, and other health-risk behaviors. The survey is completely anonymous and confidential. Students may skip any particular question(s) they choose not to answer. Participation is voluntary. If you do not want your student to participate, please complete the CHKS Passive Consent Withdrawal form and return to the administration office. Forms are also available at Centennial and were mailed home over the summer. A copy of the survey is available for preview in the lobby at Centennial.
